VE Manual : 8.1 Applications for New/Upgraded Licenses
VEs are required to send us the completed NCVEC Form 605 of any applicant who earns a new license or who upgrades, along with a copy of any document (such as a copy of their current license or copy of a CSCE) used for exam element credit and all answer sheets. NCVEC Form 605 of applicants who fail all exams are returned to the examine. All other papers associated with a passed or failed exam element must be retained by the test team for a minimum of 15 months in the event they are needed by the VEC or FCC. (See Part 97.519(b)(3)

The documents submitted for each applicant earning a new license or upgrading consist of a fully completed NCVEC Form 605 and its attachments. There are two categories of attachments:

(1) Element credit documents (if any) stapled to the back of Form 605, such as:

A photocopy of a current amateur license

A photocopy of any CSCE(s)

(2) Examination documents stapled to the back of NCVEC Form 605

Written exam answer sheets with Exam Series # written at the top
