| VEs are required to send us the completed NCVEC Form 605 of any applicant who earns a new license or who upgrades, along with a copy of any document (such as a copy of their current license or a copy of an CSCE) used for exam element credit and all answer sheets. NCVEC Form 605 of applicants who fail all exams are returned to the examine. All other papers associated with a passed or failed exam element must be retained by the test team for a minimum of 15 months in the event they are needed by the VEC or FCC. (See Part 97.519(b)(3)
The documents submitted for each applicant earning a new license or upgrading consist of a fully completed NCVEC Form 605 and its attachments. There are two categories of attachments:
(1) Element credit documents (if any) stapled to the back of Form 605, such as:
A photocopy of a current amateur license
A photocopy of any CSCE(s)
(2) Examination documents paper-clipped to the back of NCVEC Form 605
