Filling out the NCVEC Form 605 was covered in Chapter 4. Be aware that there is also an FCC Form 605 which may used by the amateur community for applications not involving license examinations. The FCC's version of the Form 605 may not be used to apply for any function handled by a VEC including name, address, systematic call sign changes, renewals and amateur examinations and upgrades. The FCC Rules (See Part 97.17(b)(1) permit the VE/VEC community to create their own application forms. A copy of a properly completed NCVEC Form 605 may be found here.
