VE Manual : Attachments to the NCVEC Form 605

The current license

A photocopy of the applicant's current amateur license must be stapled to the back of the completed NCVEC Form 605 (see exception) since this license is used as credit for examinations already passed. An applicant may obtain license duplicate copies by writing the FCC Consumer Assistance Branch, 1270 Fairfield Road, Gettysburg, PA, 17325-7245, telephoning 1-888-CALL-FCC (1-888-225-5322.), or by filing for a duplicate license on the W5YI-VEC website, or the FCC ULS site.

Most VE teams will also accept license copy print-outs from the various online amateur service databases (such as the QRZ, WM7D, ARRL or Buckmaster websites) as evidence of holding a current amateur service license.

EXCEPTION: If the applicant presents a CSCE indicating a recent license upgrade but has not yet received the license from the FCC, you may test the applicant and forward the application to VEC without the license photocopy. (Complete details are given in Chapter 4.)

CSCEs and other exam credits

If a CSCE from a prior examination session was used by the applicant for credit for one or more exam elements, a copy of the CSCE must be stapled to the back of NCVEC Form 605.

Answer sheets, etc.

The applicant's original answer sheet(s) for written exams with a passing grade and a copy of the CSCE issued must be paper clipped to the back of NCVEC Form 605 (also include any scratch paper(s), if used). Answer sheets of examinations that do not result in a license are not submitted to the W5YI-VEC Office and must be retained by the exam team for a minimum of 15 months. Applications for new or upgraded amateur licenses submitted without applicant written answer sheets will be returned to the VE team without VEC/FCC action.

IMPORTANT: Be certain that the applicant's name and the date appears on all answer sheets. It is also desirable to have the VEs who administer the examination sign or initial the answer sheet.
