Recount the successful applications
Does this figure match the APPLICANTS UPGRADING line on the Manifest Do NOT send a NCVEC Form 605 application unless an FCC license is to be issued.
Attach a check for W5YI-VEC
Is a personal check made out to W5YI-VEC paper clipped to the front of the Manifest for the amount of test fee calculated on the Manifest The TOTAL APPLICANTS line on the Manifest is multiplied by the current year exam fee split.
Recheck the NCVEC Form 605
Are all the credit documents (if any) for each applicant attached to each NCVEC Form 605, including a copy of any current license, and a copy of any CSCEs See Technician License Credits and Element Credit section for details. Also See Part 97.505.
Is the applicant's license stapled to the back of the NCVEC Form 605 facing out along with copies of any CSCEs used for element credit Are all test papers for each passed written and/or code element paper-clipped to the back of the 605 Is the "operator license qualified for" properly checked with the correct class of license? The NCVEC Form 605 should not be sent to the VEC Office if the applicant did not did not qualify for a new or upgraded license..

IMPORTANT: It is the VE team responsibility (the three VEs who sign the NCVEC Form 605 -- not just the CVE) to:

certify that an applicant is qualified for a new or upgraded license and |

to ensure that the appropriate proof of qualification documents (either element credit documents or graded answer sheets) are stapled to the Form 605. |
